Good afternoon!  I am sure that you are very busy.  I wanted to speak to you before sending this email. 

We are going to have to cancel the SPAM LTD plan effective midnight November 30, 2021.

SPAM will need to inform each insured member of the termination.  The troopers do have the right to convert their coverage.  They would have to contact Guardian to convert their coverage.  I am attaching a copy of the LTD conversion request form.  Members have 31 days from the date the plan cancels to apply for conversion.  Conversion will be more expensive than the group plan that they currently have.

For members that have been covered by the policy, if they are disabled prior to December 1, 2021 they will still be considered for LTD benefits.  Members that are currently being paid for LTD benefits will continue to be paid, as long as they continue to be determined to be disabled.

I can provide you the list of covered members.  I would suggest that a SPAM-O-Gram be sent out so that members are aware of the termination of the coverage. 

I will also be reaching out to Jeanine Hopkins in payroll to let her know of the decision and ask that she stop the payroll deductions for the troopers enrolled in the plan.
