All members civilian and sworn:

 

DPH HAS CHANGED THE PRE-VACCINATION CONSENT FORM FOR ALL MEMBERS GETTING FIRST OR SECOND COVID VACCINATIONS – SEE ATTACHED “Covid Vaccination Consent Form” – Please print this out and bring to the vaccination site. We will have copies of this form at the vaccination sites as well.

 

DPH and the Massachusetts Immunization Information System have also added new personal information fields that must be filled out when you check in at the IMAT desk – Gender, Race and Ethnicity will be added during your check-in

 

If you signed up for the vaccination on Fri 1/29 and received the “Thank you – your response has been saved” message the you are good to go – NO EMAIL CONFIRMATIONS were sent from this form.

 

Instructions and directions for the vaccination sites are attached to this email– GHQ ONLY for the vaccinations on 1/29/21
