I thought this was all set.  I spoke with Favour Fawale.  She said we should have a receipt if this was paid online.  Did you happen to get a receipt.  If this did not go through, it can be paid by check (made payable to the Commonwealth Print Service).  Thank you.

Payment can be made online at MAOSDBillPay.com. If you prefer not to pay online, please provide a completed W-9 so we can process the billing accordingly.
