I am attaching the form that we put together on our end. The intent would be that the trooper would sign this form and we could continue to update deductions/age bracket changes on an annual basis until a trooper would like to stop the deductions. The deduction termination form is from the payroll area.
Can we see how payroll feels about that?
We would need to let payroll know that SPAM would notify troopers via a SPAM O Gram that the changes are going to happen. So my though is that you could send a SPAM O GRAM to all the trooper to let them know that the change will happen each year.
